Becoming a New Client

When you contact Twomey Speech & Language Services by phone, you will speak directly to Karen who will answer any questions you may have about the evaluation or therapy process. This is also a good time to briefly share the concerns you have for your child. If you are interested in seeking services you may schedule an initial consultation. Karen will ask you to download, read over, and complete important forms to bring with you to this first meeting. It may be most appropriate to fax or mail these forms before meeting in order for Karen to begin to understand your child’s needs. If you have insurance coverage through Blue Cross / Blue Shield of Massachusetts, Tufts Health Plan, Harvard Pilgrim Health Care or MassHealth; Karen will ask for your insurance information over the phone. Although Karen is not affiliated with any other insurance carriers at this time; upon request, she will provide you with the necessary documentation for you to submit to your insurance company for reimbursement. Twomey Speech & Language Services’ fee schedule will be explained and you will receive a written copy.


  • Case History

    The Case History Form asks you to describe the concerns you have for your child. It is a means to obtain pertinent background information: including your child’s early speech and language milestones; developmental, social, behavioral, and medical information. The case history is one way the speech-language pathologist learns about your child, and it assists in the evaluation process as well as developing a therapy treatment plan.

  • New Patient Intake-Consent

    The Intake and Consent Form provides identifying information about you and your child, health insurance specifics, and it requires your signature granting permission for a speech-language evaluation and/or therapy services.

  • Notice of Privacy Practices

    This notice describes the privacy practices of Karen W. Twomey, SLP of Twomey Speech & Language Services for clients receiving services according to the Health Insurance Portability and Accountability Act of 1996, known as HIPAA.

  • Privacy Practice Acknowledgement

    By signing this form, you acknowledge you have received and reviewed the Notice of Privacy Practices and your questions have been answered to your satisfaction.

    Signing this form allows Twomey Speech & Language Services to release and/or exchange your health information with other individuals or agencies in any situation that is not described in the Notice of Privacy Practices.
